Your referral form has been submitted. Here's what happens next. 

One of our team will contact the applicant to help them complete the full application form. 

There are number of ways we can help them register.

• The quickest is via our online form
• Over the phone with one of our team
• Printed application which can be posted or downloaded

Please note as part of the main application process we will need the following documents to confirm their service record and assess whether they meet our sight loss criteria:

• Service papers
• Eye test report
• Certificate of visual impairment (CVI)
